Enrollment at Taft Middle School

We have a convenient enrollment process that allows you to complete all the forms online. All forms should be sent electronically to Marsha McNeil at mmcneil@sandi.net

There are 9 items that need to be sent to Marsha McNeil to complete the enrollment process. Items 1-4 are fillable PDF's that need to be downloaded, completed, and then sent in. A copy of items 5-9 need to be scanned and sent in.

  1. 2020-21 PreK-Grade 12 ENROLLMENT FORM (Click Here)
  2. HOME LANGUAGE SURVEY FROM (Click Here)
  3. HEALTH INFORMATION EXCHANGE CONSENT FORM (Click Here)
  4. SDUSD California Immunization Registry (CAIR) FORM (Click Here)
  5. BIRTH CERTIFICATE
  6. IMMUNIZATION RECORD
  7. LAST REPORT CARD
  8. PROOF OF RESIDENCY (copy of a bill @ SDGE, cable, water, lease agreement)
  9. A copy of student IEP, if applicable

You will receive further instructions once these forms are completed and submitted to Marsha McNeil at mmcneil@sandi.net. Please email mmcneil@sandi.net if you have any questions about the enrollment process. 

**NEW TO THE DISTRICT

Only students who are enrolling in school for the first time or are new to the district (some criteria applies). This option is available in both English and Spanish. (This online registration system is separate from the Online Enrollment Options application).


Yearly Forms

The Forms listed below need to be completed yearly by each Taft Middle School Family. Please complete digital forms and email them to Marsha McNeil (mmcneil@sandi.net).

  1. Universal Form. This form can be downloaded and emailed to mmcneil@sandi.net (available below as a fillable form in English and Spanish).
  2. LCFF Form. This form can be completed and submitted online through PowerSchool Parent Portal.
